   Please complete form at least 2 weeks before expiration of your ID Card. Make sure to include all vehicles you intend to use when visiting CAP.     Please print, sign and return the completed form to the Admin Officer or the Squadron Commander at CAP. You will be taken to the form in 8 seconds.
